The episode showed a cartoon version of Trump in bed with Satan, and Satan made a joke about the size of his penis. Later in the episode, there was a live-action deepfake segment where a naked Trump wanders through the desert, and a voiceover states, “His penis is teeny tiny, but his love for us is large.”

Creators Trey Parker and Matt Stone have even commented on the decision to show it unblurred. They reportedly refused a request from producers to blur the animated depiction, and to get around it, they added eyes to the penis, effectively making it a “character” in their view.

This was certainly one of the elements that drew strong reactions and made the episode widely discussed, including a response from the White House.
